In the meantime, this page should give you an idea of what you need to do to prepare for your visit to make sure it goes as smoothly and productively as possible.

Your First Visit

Our front desk staff will give you a friendly greeting and give you the necessary paperwork to fill out before your appointment. These forms will let us know if you’re suffering from any specific health issues that need to be addressed or taken into account. Once Dr. Whitmore is ready to see you, you’ll receive a routine examination followed by a thorough cleaning of the teeth and gums or treatment plan. We’ll schedule future treatments based on what was found.

New Patient Forms

There’s no need to wait until you get to our dental office to fill out the necessary forms; you can download them and print them out right here so that they can be completed with all of your information ahead of time. This simple step will dramatically cut down the amount of time spent in our dental office and allow you to see Dr. Whitmore that much sooner. Remember: these forms are absolutely vital for letting us know how to approach your care.
